HOW TO REPORT A POTENTIAL ENVIRONMENTAL VIOLATION IN EPA REGION 10

1. If the violation is in EPA Region 10 (Oregon, Washington, Idaho, or Alaska), call (206) 553-4973 or (800) 424-4372 and ask a staff member to complete a complaint form for you. The Regional "tip and complaint" reporting service is Regional only, and intranet based.

2. If the violation is in any EPA Region, including Region 10, go to www.epa.gov and click on the Gold Shield. The completed form will be forwarded to the appropriate contact at either an EPA Region or at EPA's D.C. Headquarters office.
